We are hiring IMMEDIATELY for a Clinical Counselor to join one of our community mental health providers!

Summary:

Participate in a multi-disciplinary team providing program specific services to individuals and/or families served. Ensure that the Individual Service Plan meets client needs. Develop and maintain an on-going supportive relationship with the individual(s) served, as well as community partners also involved in the client's service plan.

Essential Functions/Major Responsibilities

• Maintain regular contact with the individual, family (or identified support system), and representatives of other systems in which the client is involved.
• Provide case management services as needed. Must be able to identify when the client is in crisis, follow crisis response guidelines, and inform relevant contact for necessary intervention.
• Maintain the individual’s case file, gather and transmit fidelity measures, and assure that they are submitted according to protocol.
• Collaborate with the multi-disciplinary team in developing and implementing a transition plan from one service level to another.

Minimum Education Level

Bachelors

Specifics: In behavioral science field.

Minimum Experience Level

Two- Three years

Specifics: clinical/treatment experience in behavioral health field.

Certifications:

Specifics: Classified as a Qualified Mental Health Associate.
