HOAP Case Manager
Site Location: HOAP | 694 Church St NE, Salem, 97301
Position Status: Full Time | Mon -Fri (8:30 am - 5:00 pm)
Sign-on Bonus: $1000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)
Referral Bonus: $500 (Referral bonus’ expire at the close of this job posting)
YOUR ROLE:
Through positive support, motivational techniques, and encouragement, you will provide strength-based case management to houseless adults seeking support through our HOAP Program. You will work closely with HOAP guests to evaluate needs and develop individualized service plans to help them achieve their health, housing, education, and employment goals. As a member of the Care Coordination team, you will work alongside a multi-disciplinary team of mental health and medical professionals to address barriers to positive health outcomes. Through our Victims of Crime Act (VOCA) grant funding, you will develop relationships and advocate for crime victims using evidence-based approaches, providing information about VOCA Programs in an effort to coordinate care and connect each individual with appropriate services. This position also supports the HOAP Day Center operations, by providing floor coverage and drop-in support.
A trauma informed and collaborative approach will be keys to success in this role. You will have opportunities to make a significant, positive impact as you work alongside agency team members, and community partners in our shared goal of stabilizing and improving the health and social outcomes of our clients.
QUALIFICATIONS:
- A Bachelor’s degree in a behavioral science field OR a combination of at least three years’ relevant work, education, training, or experience AND demonstrated competency in effective communication; understanding mental health assessments; treatment and service terminology and application of these concepts; provision of psychosocial skills development; implementation of interventions as assigned on an individual plan of care; and provision of behavior management and case management duties.
- Minimum two years’ experience working in mental health and/or with vulnerable populations. Familiarity with mental health conditions and medications
- Oregon driver's license and auto insurance (this position requires driving and a motor vehicle report is conducted prior to hiring)

The Homeless Outreach and Advocacy Project (HOAP) has been in operation since 1986 and offers essential day center, case management, and medical services for adults experiencing homelessness. HOAP operates under a low-barrier treatment model that affords at-risk individuals with a more flexible and holistic approach to their care. The HOAP Day Center serves as the entry point for building trust and lasting relationships by providing a space for safety and support. By ensuring continued access to both medical and wrap-around support services, vulnerable individuals are provided with a better opportunity to maintain their long-term health needs and housing goals.
