Position Title: Case Manager—Enhanced Case Management
Reports To: Behavioral Health Clinical Manager
Class: Full-time, Non-Exempt Position
The ECM Case Manager is responsible for outreaching and enrolling clients in enhanced care management. They will work collaboratively with and as part of the ECM interdisciplinary team to provide high quality, effective case management to ECM members. Case management includes, but is not limited to, comprehensive assessment of needs, development of a care plan, provision of intensive case management services, use of therapeutic interactions to promote health behaviors, coordination of care, and resource linkages to medical, psychiatric, behavioral, educational, and other services as needed. The ECM Case Manager will complete assessments and scales as appropriate and will be responsible for ensuring all documentation is completed in a timely manner.
Essential Duties and Responsibilities
- Conduct outreach, enrollment, and case management to selected population of focus determined by assigned health plan and facility.
- Develop an individualized comprehensive management care plan integrating clinical and non-clinical needs to achieve health goals designed to improve functional status, health status, or prevent decline.
- Manage and coordinate care for a defined caseload of ECM members with complex needs.
- Engage with members both in person in a field-based setting and on the phone in a manner that utilizes evidence-based approaches, such as MI, that promote collaboration between the member and his or her health.
- Assists with the coordination of medical and behavioral health access issues.
- Participate in outreach and engagement efforts to enroll referred members in ECM.
- Utilize brief behavioral health therapeutic interventions as necessary to improve the member’s ability to manage their own mental health concerns and symptoms.
- Attend any health fairs, events, and other activities as assigned.
- Maintain documentation for each contact as instructed and within the program timelines (weekly, monthly, quarterly).
- Develop relationships with outside organizations and individuals for the ECM program.
- Display knowledge of other services offered at Families Together, such as: Medical, Mental Health, Dental, Vision, Physical Therapy, Nutritionist, etc.
- Maintain knowledge of the different programs available at FTOC, such as: Family PACT, Every Women Counts, Tile X, CHDP, etc.
- Behave professionally and work collaborate with co-workers to increase productivity.
- Understand the organizations chain of command and report all issues, concerns, suggestions to direct supervisor.
- Perform other project duties as assigned.
Position Qualifications, Skills and Experience
- At least 2 years working for a community based organization or community health center.
- Knowledgeable and skilled in evidenced based communication such as Motivational Interviewing or similar empathy-based communication strategies.
- Deep understanding and knowledge of mental health, substance use conditions, and homeless resources.
- Strong Case Management experience.
- Knowledge and experience in working with diverse communities.
- Highly skilled interpersonally, with excellent relationship skills including resolving conflict with co-workers.
- Resourceful community liaison experience navigating through complex health systems and community services.
- Must be able to work as a member of a highly autonomous team, executing job duties and making skillful decisions as an independent team.
- Strong assessment, interpersonal, and intervention skills.
- Strong writing and oral presentation skills.
- Exceptional organizational and planning skills.
- Skilled in meeting project deadlines.
- Great time management skills.
- Ability to work as a part of a team is crucial.
- Available to work weekends if needed.
- Effective communication skills.
- Familiarity with electronic medical records.
- A valid CA driver’s license and clean driving record
- A reliable vehicle and valid automobile insurance
Education and Training
- Bachelor’s degree in social work, psychology, counseling, sociology, or behavioral science
- Proficient in MS Office and Outlook
- Bilingual English/Spanish (strongly preferred)

Pay: $25.00 per hour
