Salary Description: Minimum Salary: $25.52+ per hour
Department: Case Management- Schedule: 2x12s Saturday and Sunday 7am-7pm or 8am-8pm
- Hospital: Ascension St. Vincent's Southside Hospital
- Location: Jacksonville, Florida
Support various services in case management including collaborating with the care team and educates patients or their caregivers as to the case management process.
- Utilize negotiation skills to secure appropriate options and services necessary to meet the patient’s benefits and/or healthcare needs.
- Facilitate information and support to empower the patient to make ongoing independent medical and/or healthy lifestyle choices. Identify and document all referrals to appropriate facilities or agencies.
- Expedite discharges by transmitting appropriate documentation to providers for the acceptance of patients. Document all interactions with payers and communicates status with appropriate staff.
- Identify and escalate quality of care issues. Provide status updates through established channels with the case management staff. Liaison between case management, admission, payers, etc.
- Advocate for resources and removal of barriers.
Licensure / Certification / Registration:
- Social Worker preferred.
Education:
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
