Housing Works was founded in 1990, with a long-term commitment to AIDS advocacy. Housing Works established New York State's first harm reduction-based, OASAS-licensed outpatient drug treatment program. Other services include Health home care management, behavioral health, and syringe exchange programs.

A pioneer in the social entrepreneurship movement, Housing Works operates 10 high-end thrift shops in Manhattan and Brooklyn and a much-loved Bookstore Cafe in Soho.

The Role

  • Take a lead in assuring that clients sustain connection to life-saving services and play an active role in the recovery and wellness of the people we serve.

Care Navigators are the main link among clients, agency staff, and medical or psych care providers.

  • Conduct general assessments, enrolling clients into case management programs, providing health education and various other services.
  • Promote, the recovery, independence, health and wellbeing of service users. Will work actively with team members and other organizations/resources to assure appropriate service provision is provided to our population.
  • Navigators must be knowledgeable about the communities served, the problems that impact their lives and its available resources. Including educational, social and emotional supportive services available to clients.

Compensation
$19.00/hour- $22.00/hour commensurate with experience

What You Will Do

  • Target case finding either by engaging other organizations, providers, or programs servicing the populations served by the program.
  • Conduct initial assessment with each participating consumer to assess his/her health and wellness status and experience with getting health care.
  • Health promotion, health education, and risk reduction counseling.
  • Maintain routine communication and assist with coordinating the Client's navigation and goals with appropriate agency staff routine communication with all members of the care team.
  • Communicate with clients as indicated by the clients established service plan to keep them engaged and to assess progress as well as tracking the progress of clients including conducting follow-ups.
  • Navigate the medical system including such things as helping the clients make medical appointments, going with the client to appointments, helping the client communicate with medical personnel and/or talking to clients about ways to communicate with medical personnel, helping with any follow-up necessary.
  • Complete and submit documentation of services provided in a timely and efficient manner (e.g., within two business days past the provision of service).
  • Participate in the advocacy of clients and organizational events as warranted.
  • Conduct home visits for current participants as needed.
  • Maintain connection with care providers and partner agencies, promoting adherence to medical regimens, plans of care and achievement of self-management goals.

Who You Will Be

  • Someone with a minimum of a high school diploma or General Equivalency Diploma required.
  • AI Peer Certification (or pending certification) also accepted.
  • Bachelors in human services or similar disciple, preferable.

Job Type: Full-time

Pay: $19.00 - $22.00 per hour
