Long Island Adolescent and Family Services (LIAFS) is a not-for-profit Social Service agency that provides residential and ancillary services to male and female children and youth. With four sites in Suffolk County, our multi-faceted services include: casework and mental health counseling, medical and dental care, independent living skills training, education and recreation. In addition to our work, we also offer services to families in need of support and assistance
Care Manager – (F/T) Monday - Friday ( two evenings) Provide care coordination that is youth and family driven, supports a system of care that builds upon the strengths of the youth and family. Integrate the delivery of physical and behavioral health services, identify and address needs early to prevent escalation and longer term need for increased services. Maintain youth in the community and at home with supportive services. Collaborate with families, other collaterals and schools and oversees the coordination of access to all services (behavioral, medical health and social service supports) to assist and support youth . BA and exp. in care management required. CANS pref. Competitive salary and benefits.
