Bilingual Program Assistant Job at Kalihi Palama Health Center
Primary Purpose:
The person in this position is a member of the Patient Centered Medical Home where each team member plays an integral role in providing patient-centered health care. The incumbent is responsible in assisting the PCMH team in population management, facilitating referrals follow up, managing care opportunities, and documents necessary for an efficient & enhanced patient centered care.
Essential Duties:
- Assists in population management activities, which includes but not limited to contacting and scheduling patient appointments, reminder calls, sending letters etc.
- Assists in coordinating referral, emergency department and hospital follow up
- Assists in managing disease specific care opportunities using health plan data
- Assists in the completion of various forms such as EPSDT, MDX, OPTUM etc.
- Assists in coordinating Hepatitis B/Newborn screening as necessary
- Takes images for Diabetic Retinal Exams
- Creates & organizes medical assistants scheduling
- Ensures timely patient follow up care as necessary
Enters and tracks data in EMR or other population management tool in a timely manner as required
Regular and reliable on-site attendance is required.
Other Duties:
- Participates in care team meetings
Perform other related duties as assigned
Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
Minimum Qualification Requirements:
Education:
High School Diploma required and medical assistant certification preferred
One year certificate from an accredited Medical Assistant program
Experience:
1 year general experience in working in a community or hospital setting preferred
Must obtain MA certification within 12 months
Skills/Knowledge:
Medical terminology proficiency required
Computer proficiency required
Ability to type 60 words per minute
Ability to add, subtract, multiple and divide in all units of measures, using whole numbers, common fractions and decimals
Ability to compute rate, ratio and percent and to draw and interpret graphs
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form
Ability to deal with problems involving several concrete variable in standard situations
Bi-Lingual in English and either Cantonese or Mandarin required
