What you can expect:

Reporting to the Clinical Director, the Assertive Community Treatment (ACT) program is an intensive and highly integrated approach for community mental health service delivery. As ACT Team RN you are assigned to individuals with severe and persistent mental illness, typically with a history of lengthy or frequent hospitalizations and/or a need for intensive community supports provided on an outreach basis. Under this model all ACT team members share the responsibility and welfare of consumers who are assigned to the program with the goal being to maintain the individual in the community and reduce hospital recidivism.

How you will make an impact:

  • Serves as part of team providing a therapeutic set of intensive clinical, medical, and psychosocial services that includes case management, psychiatric services, counseling/psychotherapy, housing support, substance use disorder treatment, and employment and rehabilitative services in community settings.
  • Attend daily team meetings to review problems and plan solutions.
  • Position requires travel (agency vehicle available).
  • Provides weekly community-based interventions to individuals.
  • Provide transportation assistance to individuals served.
  • Serves as liaison to the staff psychiatrist/s in monitoring customers who are taking prescribed medication. Such monitoring will include Vital signs, AIMS screening, PHQ9/PHQA, Columbia Suicide Severity Scales, psychoeducation of disorders/medications, and medication consent, evaluation for medication side effects, physician coordination letters to primary care physicians/pediatricians and specialists as applicable.
  • Provide nursing services to customers as needed, including education, linking and advocacy for completion of medical needs.
  • Administers medication as ordered by staff psychiatrist to customers, as applicable, including oral, topical, sub-Q and IM injections.
  • Coordination of health and safety training for agency staff post hire and ongoing as requested. Monitors/administers TB, Hep B vaccinations.
  • Provides education, referral and monitoring of health and wellness needs of consumers, education to staff and primary care providers regarding unique needs of CMH consumers.
  • Participates in regularly scheduled supervision sessions with supervisor.
  • Act as liaison between CMH and local medical providers to assure that individuals served are receiving integrated health care, having preventative care completed, at times of concerns regarding medications, nurse to nurse conversations for coordination of care (including transition services). Link with Cass Co Jail nurse as applicable when needed.
  • Completes all required documentation completely, accurately, and timely
  • Completes After hours hospital pre-screening assessments.
  • Utilizes a Trauma-Informed approach to deliver services.
  • Establishes collaborative relationship with Individuals.
  • Matches specific supports and interventions to the unique needs of individual Individuals and recognize the importance of friends, family, and community relationship.
  • Enhances the ability of the Individuals to lead a self-determined life by providing the support and information necessary to build self-esteem and assertiveness, to make decisions, and improve their quality of life.
  • Rotation for afterhours crisis coverage (on-call).

Minimum Education & Experience Requirements:

  • Graduate from accredited school of nursing, diploma, associate degree or B.S.N./M.S.N.
  • Valid State of Michigan RN license required
