The Admission Clinician Hampstead actively responds to all admission inquiries from both individual and professional sources. This position educates prospective patients, their families and community-based practitioners to hospital treatment programs and services, and admission criteria for inpatient level of care. The Admission Clinician performs Level of Care Evaluations and ensures insurance authorization or alternative payment for services is in a place prior to patient admission. This position works collaboratively with medical, clinical, and unit-based staff to review and schedule new patient admissions when feasible and works closely with community-based referral sources to ensure all necessary referral information and documentation is provided.
Qualifications & Requirements:
Education
- Master’s degree in Social Work or Counseling from an accredited graduate program, or graduate of an accredited school of nursing with current RN NH licensure.
Experience
- Prior experience working in a hospital or psychiatric setting
Licenses/Certifications
- Eligible Social Workers encouraged to complete LICSW certification process
- Eligible Master’s prepared Counselors encouraged to complete LMHC certification process
- CPR Certification (within 3 months of hire)
- CPI Certification (within 3 months of hire)
Compliance
- Corporate Compliance (within 2 months of hire)
KEY JOB RESPONSIBILITIES:
- Provide clinical assessments for patients to determine level of care.
- Educate prospective patients and their families to hospital treatment programs and services, hospital level of care criteria, health insurance coverage and/or other payment procedures.
- Complete telephone intake with prospective patients and schedule level of care evaluations when feasible.
- Conduct telephone intakes to include obtaining information regarding reason for referral, clinical presentation, level of functioning, demographics, living situation, and insurance coverage.
- Accept referrals from community emergency services providers and other practitioners including gathering all necessary demographic and clinical information needed to present to medical staff and/or the treatment team.
- Accept patients for admissions as appropriate. Verbally present medically or clinically complicated referrals to the doctor on call and/or treatment team for their determination to admit or decline patient.
- Complete referral process by communicating either acceptance or denial of referral, procuring insurance authorization information, or alternative funding information and scheduling time of patient arrival.
- Coordinate pending admissions with clinical, supervisory, and unit-based staff to include updating anticipated time of arrival, providing available clinical documentation, and altering to meet special treatment considerations.
- Manage the daily admission flow including scheduling times for level of care evaluations and direct admissions.
- Respond to all incoming admission inquiries in a timely, efficient, and professional manner.
- Facilitate the signing in of new patients by reviewing and completing all required Admission documentation in accordance with Joint Commission, State Licensing, and Medicare standard requirements.
- Promote the hospital culture of patient safety in a non-punitive atmosphere, immediately reports medical errors and significant events to the nurse in charge; participates in hospital safety training programs and Performance Improvement activities.
