Position Description
OBJECTIVE:
To assist Day Program staff in effectively implement Individual Plans (IPs) and program techniques through community inclusive activities.
QUALIFICATIONS:
High school graduate (GED). Medication Administration Qualification {or complete training within 30 days}. CPR/First Aid training or willingness to obtain training. A valid Colorado driver’s license without a history of violations and successfully completing the company’s vehicle certification program is also required.
OTHER REQUIREMENTS/PREFERENCES
Previous experience with mild to severe profound developmentally disabled adolescents and/or adults helpful.
It is preferred that the incumbent have the ability to interact, work cooperatively and effectively. The incumbent must be able to respond to special requests and be flexible in regard to job tasks and duties. The incumbent must have good physical stamina/endurance to stand for long periods of time. The incumbent must also demonstrate empathy and concern for individuals with developmental disabilities and have a passion for working to maximize their independence and personal growth. The Community Liaison must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation. The incumbent must be able to work effectively as a team member with the rest of the Rehabilitation staff.
KEY RESPONSIBILITY AREAS:
Community Integration
The Community Liaison will work with Day Programs team members to schedule community activities coinciding with curriculum. All community integration activities will be person centered related to individual goals. The incumbent will provide transportation to clients, in the company’s vehicles and supervise overall care while away from the facility. The Community Liaison will work with various community members to ensure client success in the community is fulfilling and safe. The incumbent will also assist the rehab team in planning and scheduling reverse integration activities into the facility. The Community Liaison will ensure proper documentation is completed each time Community Integration is completed. All documentation should be person centered around individual goals.
Client Care
The Community Liaison will assist in the implementation of program plans, giving reinforcers, providing one-on-one assistance and/or training. The incumbent will assist in the preparation necessary for arrival and end of day client departure and provide supervision at the beginning and end of the day to ensure safety of program participants when both exiting and entering the bus and facility. The Community Liaison will communicate pertinent information to rehabilitation staff and assist program participants with transitioning throughout the day. The Community Liaison will also assist with medication administration when needed. The incumbent will be an active member of the Day Program team and will assist team members with all client care.
ISD Team Support
As a member of the team, the incumbent will participate in staff meetings as directed. The Community Liaison will also teach proper self-help skills, and appropriate interactions through role modeling and programmatic intervention. The Community Liaison will provide coverage in the programming areas and lunch periods when at the facility.
Client/Program Tracking and Reporting
The Community Liaison will fill out all necessary progress reports per client, necessary for billing and tracking. The incumbent will assist Case Managers with completing any assessments needed on program participants.
Relationship Management
The Community Liaison will communicate in a timely manner and keep the Program Coordinator informed of progress, considerations, and pertinent information. The Community Liaison will deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required. The incumbent may be needed to communicate with parents or Host Home Providers as assigned by the Case Manager or Program Coordinator.
Pay: $15/hour
Work Schedule: Monday - Friday 7:30am - 4:00pm (flexible)
