Founded in 1971, Centers for New Horizons, Inc. (Centers) is a nonprofit organization that assists children, youth, and families to become self-reliant. With 200+ full-time staff, most of whom live in or near its service communities, Centers provides programs in seven locations throughout the Bronzeville and Riverdale communities of Chicago's Southside that reach over 1,000 individuals daily.
Position Description
A Family Youth Coach will assist youth clients in exploring their interests and abilities through a wide range of youth development, awareness and assessment activities. The Family Youth Coach will work both one-on-one and in a large group settings with youth participants, to help achieve identified social, emotional and academic goals. This position is responsible for developing and implementing youth programs and projects in conjunction with the Family Coach in a collaborative effort to help the youth and the family overall achieve their goals. The Family Youth Coach has the responsibility of planning and coordinating the youth focused activities where they must be knowledgeable of the needs of the youth and the population that we serve.

Work Environment
The typical work environment is in an office/recreation facility but may on occasion be outside the office. The work environment may have occasional exposure to disagreeable elements. Employee will generally use the telephone, computer, calculator, copier, overhead projector, paper shredder, paper cutter, and filing systems.
Supervision
Direction received from the Program Services Manager. Does not supervise any employees.

Essential Functions
- Manage a caseload of CHA youth participants
- Performs intakes, assessments and self-sufficiency planning of families and individuals.
- Responsible for planning and coordinating youth focused events and activities
- Coordinates youth activities with CHA’s Resident Services Division
- Knowledgeable of the service needs of the youth population in order to coordinate youth services for the Contractor
- Attends monthly youth liaison meetings
- Compile reports, participates in data collection
- Monitor goals set within success plans
- Track client services and record via physical and electronic formats.
- Identify and establish appropriate linkages with educational, social, and sports and recreation providers as resource options for program population.
- Independently follows agency policy and procedures
- Complete any other related tasks and responsibilities as assigned and required
- Follows through with commitments including meeting established timelines
- Identifies and resolves difficult issues
Essential Responsibilities
- Attend regularly; dependable coming to work as scheduled
- Report absences within department procedures
- Attend and participate in required meetings (i.e., all-agency meetings, department/program meetings, supervisor meetings, etc.)
- Successfully complete all in-service training required for the position within timelines and participate in other training, as assigned
- Manage work time to meet needs of internal and external customers, stakeholders and individuals receiving services
- Expands knowledge of departmental/agency operations
- Participates in performance review and personal/professional growth
Qualifications
- Bachelor’s Degree in Education, Social Work or a closely related field from an accredited college
preferred or a combination of college course work and related previous work experience, preferably in a nonprofit agency.
Required Knowledge, Skills and Abilities
- Thorough knowledge of business English, spelling and punctuation.
- Thorough knowledge of customer service, office practices, procedures and equipment.
- Proficient in Microsoft word, Excel, Access, Power Point and other computer software packages. Type a minimum of 40 wpm
- Knowledge of various population contracts
- Ability to communicate effectively and project a professional image when giving and taking information in writing, in person, and over the phone.
- Must be able to work in and independent manner.
- Ability to take initiative and prioritize tasks; good time-management, problem-
- Ability to work accurately with close attention to detail.
- Ability to maintain confidentiality of sensitive information.
