InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:
The Assessment Clinician works collaboratively with multi-disciplinary treatment team to ensure timely and accurate patient assessment and treatment planning.
Essential Duties & Responsibilities:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- As a member of multi-disciplinary clinical team, develops psychosocial assessments through clinical interviews with clients and which include consideration of substance abuse, medical, mental health, and developmental assessment, using American Association of Addiction Medicine (ASAM) Criteria.
- Uses ASAM patient placement criteria to recommend the next level of care and ensures a referral is made.
- Develops treatment plans including measurable treatment objectives.
- Provides clinical treatment services in form of triage, crisis intervention, assessment, individual, group and/or family therapy, screening and planning.
- Advocates and develops networks of social and clinical services to assist clients in meeting identified needs.
- Completes all required documentation related to client treatment in accordance with established procedures.
- Completes reports in accordance with agency and accrediting bodies.
- Consults and collaborates with other behavioral health providers to improve delivery of services to clients.
- Maintains treatment records in accordance with agency and regulatory bodies.
- Supports the utilization review team with obtaining treatment authorizations at admission to residential treatment.
*All agency staff are required to attend all mandatory department/agency meetings and trainings*
Requirements:Education &/Or Experience:
A Master's degree, Connecticut State Licensure (LCSW/LPC, LMSW, LPCA) and three (3) years relevant experience. Also requires experience working in a Recovery-Oriented System of care.
