Wellness Nurse – LPN

Hours:

We are looking for a full time Nurse for weekdays and every other weekend to help with med passes and assist with residents returning to the facility from rehab centers and hospitals.

We are looking for a reliable, compassionate, cheerful Wellness Nurse to join our team at Stonebridge of Gurnee. We are a supportive living facility assisting individuals age 22-64 with their goal to maintain independence.

Responsibilities:

• Daily documentation of dispensing controlled substances and narcotics to ascertain that they are properly administered, counted and documented properly.
• Personally receive or place calls to physicians and transcribe all physician's orders.
• Supervise all Direct Care Personnel in performing their duties by monitoring their work closely to ascertain that assignments have been completed based on assignments and the resident’s individualized service plan throughout and before the end of shift.
• Empathy while providing exceptional nursing care.
• Problem solving & customer service skills.
• Time Management skills to ensure all residents are provided the best level of care.
• Take and record measurements of blood pressure, temperature, heart rate, blood sugar, etc, as needed.
• Educate residents on self-administration of blood glucose monitoring and injections, prescribed medications, self-care, etc, and assist with other basic medical care tasks
• Collect samples for lab testing as assigned
• Provide emotional and psychological support when needed
• Familiarity with infection control, safety and sanitation standards and procedures
• Understanding of confidentiality obligations and nursing best practices

· Knowledge of Point Click Care preferred

Salary: $28.00 - $32 / Hour

Requirements:

• Current and unencumbered Illinois license to practice as a LPN
• Current BLS/CPR certification
• Ability to work independently
• Strong sense of initiative and urgency in addressing patient crises
• Electronic charting

Job Types: Full-time, Part-time

Pay: $28.00 - $32.00 per hour

Weekly schedule:

• Monday to Friday
• Weekend availability

Experience:

• Healthcare: 1 year (Preferred)

License/Certification:

• LPN (Required)
