Universal Worker/Caregiver

Position Summary: This position will offer services to residents as indicated in each resident’s individual service plan within multiple communities associated with Sage Oak Assisted Living and Memory Care. In addition, this position will assist in the development and revision of service plans and observe and report any issues or concerns surrounding residents and their care. This position will also perform certain food services, activities, environmental duties and assist with medication management.
The ideal personnel will be able to communicate and demonstrate positive company culture. Personnel will contribute their talents and energies to further improve the quality and environment of those who we care for. This position requires teamwork and critical thinking skills with a problem-solving attitude to contributing to the daily clinical operations of multiple communities. This position will serve as a role model for others by displaying a responsible, cooperative, and positive attitude with a willingness to help out when scheduled. Furthermore, this position will provide support to residents and hands-on assistance to fellow staff.

Position Supervisor: House Manager and/or Facility Manager

Responsibilities: include but are not limited to
  • Maintains the comfort, privacy, and dignity of residents in the delivery of services. Interacts with residents in a manner that displays warmth and promotes a caring environment.
  • Demonstrates a positive, cheerful attitude and treats all residents, families, supervisors, and co-workers with dignity and respect. Communication and interactions are effective and tactful.
  • Demonstrate and facilitate company philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents.
  • Responsible for administering quality care to meet the needs of each resident.
  • Supervise or assists residents with performance of activities of daily living as indicated by individual service plan.
  • Supervises or assists residents with performance of simple treatments as indicated by individual service plan.
  • Assist residents with emotional health and/or problem behavior issues as indicated by individual service plan.
  • Redirect and/or de-escalate residents when emotional and/or problem behavior issues occur.
  • Perform other services as indicated in individualized service plans (laundry, meals, and pet care).
  • Prioritizes resident care according to need and acuity.
  • Assist with medication administration, handling, supervising, and storage as follows:
    • Provides medication assistance for residents in accordance with state regulation and company policy including pouring, preparing, assisting with administration, and documentation.
    • Ensures proper handling and/or administration for residents.
    • Ensures accurate medication counts for narcotics and storage of medications in medication cart.
    • Ensure and maintain compliance with company policies and procedures related to controlled substances and documentation as such.
    • Completes medication training and/or delegation as necessary.
    • Management and/or care of special medication (injectables).
    • Demonstrates successful passing of medication to residents without error.
  • Adhere and assist in development and revision of individual service plans by communicating with Resident Care Coordinator and/or facility supervisor of residents needs and preference.
    • Observes and reports residents’ issues or problems to management and/or facility supervisor in a timely fashion.
    • Continually stays in tune to each resident’s clinical needs and acuity.
    • Identifies and follows appropriate meals and/or diets for residents.
    • Alerts facility manager and/or Resident Care Coordination if unsure to complete task as indicated by service plan.
  • Assist in maintaining the integrity of a resident's skin by ensuring completion of weekly skin assessments and completing documentation while on shift.
    • Identifies signs and symptoms of compromised skin integrity.
    • Observes and reports resident skin issues or problems to management and/or facility supervisor in a timely fashion.
    • Completes appropriate documentation about issued or concerns observed.
  • Adhere and assist in development and revision of resident preferences for each resident as needed.
    • Collaborate with facility manager and/or Resident Care Coordinator to ensure completion of resident/family preferences as needed.
  • Perform and document proper measurements of vitals on a daily basis and as need as indicated in service plan or otherwise directed.
    • Notifies facility supervisor of any needed medical supplies to be purchased.
  • Perform multiple proper transfer techniques as indicated by individual service plan for residents.
    • Gait belt, pivot, Hoyer lift, and/or sit-to-stand.
  • Communicates professionally and timely with fellow teammates, including facility supervisor, Resident Care Coordinator, outside services or vendors (Home Health, Hospice, pharmacy and etc.), and family members.
    • Effectively communicates with all residents, families, staff, visitors, and management in a positive, helpful, and productive manner.
  • Performs food service duties, including but not limited to cooking meals, serving meals, delivering meal trays to rooms, bussing tables, cleaning dining rooms after meals, and setting tables.
    • Adhere and assist with development and revision of daily and weekly menu as needed.
    • Follow daily and weekly menus provided by facility supervisor.
    • Communicates with facility supervisor before substitutions are made on daily and weekly menus.
    • Be present at mealtimes to prevent choking incidence for residents.
    • Communicates and documents appropriately with facility supervisor about meals refused or not consumed.
    • Follows plating method when preparing and presenting meals to residents.
  • Performs activities duties as assigned by facility supervisor, including leading group activities, engaging in activities with individual residents, and accompanying residents to facility-sponsored activities outside the facility.
    • Assist with development and revision of daily activities as needed.
    • Offers, encourages, and promotes activities based on activity calendar provided.
      • Offers group activity to residents, if resident refuses, offers individual activity.
    • Takes pictures of activities and/or events planned and send to facility supervisor.
  • Records incidents, errors, and accidents.
    • Reports resident concerns to facility supervisor and/or Resident Care Coordinator immediately regardless of whether resident requires medical treatment or services.
    • Ensures and adheres to incident reporting policies and procedures.
    • Collaborates with fellow staff and/or facility supervisor to observe changes in resident status, needs, or preferences.
    • Completes incident reports and appropriate documentation upon discovery of incident or accidents.
  • Performs all housekeeping duties as indicated on daily assignment sheets.
    • Assisting with environmental services while on shift
      • Sweeping
      • Mopping
      • Washing and drying resident laundry
      • Deep cleaning common areas and resident rooms
      • Cleaning restrooms
    • Performs additional environmental services as directed by facility supervisor.
  • Read Communication Binder at the beginning of each shift.
    • Reviews and demonstrates knowledge of communication provided.
    • Signs off on communications from facility supervisor and/or Administration.
  • Provides verbal communication and written report at beginning and end of shift change.
    • Gathers report the “Sage Oak Way.”
    • Documents care all provided in Observation Book.
      • Documentation is clear, accurate, and concise.
  • Attends educational and/or mandatory facility meetings monthly to ensure fulfillment of state or provincial regulations.
  • Maintains the ability to lift fifty (50) pounds or more, stand or walk for more than an eight (8) hour workday, and is able to push, pull, bend, twist, and reach multiple times throughout a shift.
  • Reports to schedule shift on time and prepared to complete task with limited attendance or tardiness issues.
  • Adhere to incontinence, toileting, reposition, and visual checks one residents during the duration of scheduled shift.
    • Provides incontinence check on a minimum of every two (2) hours or otherwise indicated by resident service plan.
    • Provides toileting assistance on a minimum of every two (2) hours or otherwise indicated by resident service plan.
    • Monitors resident’s safety by visually laying eyes each resident every forty-five (45) minimum or otherwise indicated by resident service plan.
  • Performs other tasks as assigned by facility supervisor and/or other members of management.
