Therapist - PMTO Specialist Job at Saint Francis Ministries
Ø Maintain certification as clinician, trainer, coach and reliable fidelity rating in PMTO
Ø Responsible for recruitment and retention of PMTO trainees
Ø Monitors and ensures trainees are compliant with training procedures
Ø Provides oversight needed to ensure successful implementation of PMTO
Ø Ensures the data are being collected, traced and reported monthly and accurately
Ø Ensures success and training in PMTO, as well as, fidelity to the model through effective collaboration with clinical supervisor
Ø Participates actively in training, meetings and consultations
Ø Serves as liaison across multiple levels (eg. Agency leadership, PMTO clinicians, ISII, Portal Managers, KU and community partners)
Ø Develops and prepares training and coaching in collaboration with programs at SFCS
Ø Maintains and monitors lines of communication among all relevant parties
Ø Maintains quality relationships with DCF, courts, PMTO staff and other community partners
Ø Engages children and families through various therapeutic services, including evidence based assessments, family and individual therapy, and therapeutic visitation.
Ø Completes therapeutic assessment by gathering information from the child, family, and collaterals
Ø Develops and implements treatment plans children and families.
Ø Provides intensive in-home and/or outpatient therapy to family/individual in collaboration with the case management team which could include: attendance to team meetings, education concerning early care and development, assist in forming a family support network, home-based support services and ancillary family-focused services
Ø Provides therapy to children and families, preparing them for moves, reunification or other appropriate permanency goals
Ø Facilitates and documents evidenced-based therapeutic/parenting groups
Ø Provides consultation to further permanency goals with children and families.
Ø Assists individuals/families in establishing a support network by utilizing family, kinship, and community resources
Ø Provides intervention and support for clients in crisis situations
Ø Assesses ongoing treatment and case plan goals and prepare to offer recommendations for reunification, termination of parental rights or other permanency options
Ø Prepared to make recommendations for services needed for the child and family, including mental health and drug and alcohol services, respite care and supervised or unsupervised visitations/interactions
Ø Documents all work activities/contacts with individuals/families and collaterals in agency database
Ø Responsible for creating maximum billable Medicaid hours and submitting necessary paperwork for Medicaid billing or maximum grant hours
Ø Ability to work with the court system and follow court protocols including appropriate business attire, completes reports for court, participates in court proceedings when subpoenaed, including testifying
Ø Reports critical incidents to DCF verbally within 12 hours, written within 24 hours
Ø Assists in monitoring the overall client satisfaction of the program
Ø Meets with Supervisor regularly to review assigned cases and submits reports as required
Ø Participate in team meetings and group supervision as scheduled.
Ø Implements SFCS policies and follows directives as required. Follows and adheres to all pertinent SFCS Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms
Ø Is knowledgeable of and follows all safety procedures
Ø Reports unusual incidents through appropriate Risk Management, clinical and safety channels
Ø Ensures clients’ rights are protected
