The Therapeutic Foster Care Case Manager – After Care Specialist is responsible for linking a child to all necessary medical, mental health, educational, vocational, social and support services in a therapeutic foster home setting as deemed appropriate through the coordination of Agency and the referring agency.

The Therapeutic Foster Care Case Manager – After Care Specialist works to enable the child to be maintained in the least restrictive placement and to transition back to his/her family and community of origin.

This position reports to the TFC/After Specialist Supervisor.

Education: Bachelor’s degree in social work or other closely related degree

Experience:

• One (1) year or more experience working with family systems and utilizing community resources
• At least one year of working with family systems and utilizing community resources.

Work Schedule:

This is position is hybrid with remote work options. Most of the working hours take place in the field, with only 1-2 days in office for supervision and other work-related meetings.

Must be 21 years old to apply. Pre-employment drug screen, physical and background check required.
