Community Horizons continues to grow and expand our service regions. Due to growth, we are looking to add to our team in La Crosse, WI. Would you consider yourself organized with excellent verbal and written communication skills? Are you a self-starter with the discipline to stay focused in a home office? Must be goal-oriented to meet outcomes. This is a part-time position to start roughly 20-25 hours per week however will grow into Full-Time with satisfactory job performance.

What is a role of a Support Broker?

Support Brokers are expected to provide services to individuals enrolled in self-directed programs to have the opportunity to lead a self-determined life and reach their goals. As an added layer of advocacy, working in conjunction with the support team offering available resources, and helping to navigate systems. In this role, you are expected to complete assigned tasks with the participants rather than for the participants which will be carried out through the following responsibilities.

· Participate in new referral/home visits based on members' needs and availability

· Document services and supports provided otherwise known as case notes

· Assist members with developing networks of support and resources available to them

· Work with members on the recruitment of self-directed caregivers

· Attend training, conferences, and company meetings as required

· Provide backup to the team as needed

· Additional duties as assigned

This position is a semi-remote position which means you work from a home office with 75% travel expected within the assigned region. This is a flexible schedule position however you are expected to be available during normal business hours and possibly some evenings depending on member availability.

To qualify for this position you will have;

  • A minimum of 5 years of experience working with the population of frail elderly and/or individuals with disabilities
  • Ability to manage a caseload effectively giving individual attention to each member
  • Knowledge and experience in recruiting within the healthcare field required
  • A clean background check required
