Evolve Therapy Services is a therapy management organization. We are looking for compassionate Speech Language Pathologist, to give quality care for one of our AMAZING affilited SNF facilities, Champion Care located in Antigo, WI.

Job Position: Speech Language Patholoist for an In-house Skilled Nursing Facility *Must have active Wisconsin state Speech Language Pathologist license

Job Type: Full Time to be split between 2 locations / Includes $5,000 Sign on Bonus

Job Location: 729 Park St, Antigo, WI 54409 and Nu Roc Ln, Blackwell, WI 54541

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Current State license (or is license-eligible) in state of practice
• Ongoing continuing education and professional development to maintain current licensure and certification - Evolve Therapy Services provides unlimited CEU package through MedBridge partnership
