POSITION SUMMARY:
The LWAHP Social Worker works within the Living Well at Home Program to provide mental health and substance use evaluation, referrals, and support for elders who are homeless, formerly homeless or at risk of becoming homeless.
This position requires assertive outreach, advocacy, system navigation, and service coordination to manage barriers that may impede patients' ability to secure and maintain housing. Excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively are key requirements of the job.
Clinical experience working with populations impacted by substance use disorders, and undiagnosed/untreated behavioral health is required as well as knowledge of relevant treatment systems. This position supports older individuals across Greater Boston, and although the majority of work can be completed in an office setting, it may require some in-person or community visits. The ideal candidate will be independently licensed with supervisory experience.

JOB REQUIREMENTS
EDUCATION:
- Master's Degree in Social Work and independent licensure (LICSW or LCSW)

EXPERIENCE:
- 2-4 years of experience in a clinical setting
- Preference for clinicians with experience working with patients with mental illness, substance use disorders and experience homelessness
- Experience working in systems that serve complex and vulnerable populations
KNOWLEDGE & SKILLS:
- Strong behavioral health evaluation skills including risk and diagnostic assessments
- Strong understanding and familiarity with levels of care and how to navigate behavioral health system
- Extensive knowledge of healthcare systems and relevant community resources
- Sensitivity to ethnic, cultural, gender, and sexual orientation diversity, values, beliefs, and behaviors
- Experience with client centered practices including harm reduction approach and motivational interviewing
- Must display the use of appropriate independent judgment to determine appropriate course of action in resolving problems and in emergencies related to behavioral health needs of patients
- Ability to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary, culturally diverse setting with clients, families, staff and agencies

ESSENTIAL DUTIES/RESPONSIBILITIES:
- Complete tasks related to behavioral health system navigation. Including but not limited to:
- Holistic and comprehensive biopsychosocial assessments
- SUD referrals (detoxes, TSS/CSS/residential)
- Specialized clinic referrals
- Coordination with psychiatric emergency services
- Direct client engagement and collaboration with care team for purpose of assessment and system navigation
- Conduct short term individual therapy for elders with undiagnosed/untreated behavioral health issues
- Organize and facilitate peer support groups with participation of ELAHP clients.
- Negotiate and create opportunities for access to treatment with patient's participation; advocate on behalf of patients to help decrease barriers to care
- Independently address client's behavioral health crises by completing risk assessments, referring to appropriate levels of care, developing crisis treatment plans, and consulting with program director when clinically indicated.
- Monitor clients utilization of care across the care continuum, including Emergency Department Visits and inpatient admissions
- Develop working relationships with collateral resources and other community service providers
- Maintain current working knowledge of behavioral health resources and referral pathways including upkeep of resource list
- Participate in all training requirements and opportunities as designated by program director
- Perform outreach to clients
- Actively participate in growth and development of program
- Take lead on clinical needs of program in absence of program director and when indicated
- Complies with established metrics for performance and adheres to documentation and work flow standards including clearly documenting all progress and submitting required data within expected time frames
- Utilizes the electronic health record and other IT interfaces as necessary to track and monitor patients
- Maintains HIPAA standards and confidentiality of protected health information.
- Adheres to departmental/organizational policies and procedures.
- Complete other duties assigned by the Director of Elders Living At Home Program
