Cape Regional Health System is an integrated healthcare delivery system serving residents and visitors throughout Cape May County. We seek to hire an experienced Acute Care Social Worker/Discharge Planner who will embrace our mission to provide the highest quality healthcare to our community.

As the Inpatient Discharge Planner, you will:

• Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization.
• Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers.
• Complete comprehensive psychosocial assessment and review clinical needs of patients as they transition to the next level of care.
• Collaborate with physicians, nurses, and the care team to foster a coordinated approach to discharge planning.
• Utilize clinical knowledge to participate in treatment plan discussion with clinical teams.
• Identify and connect patients to services and resources available to them.
• Communicate with Utilization Review staff on any denials, issues, or barriers to discharge.

Qualifications Include:

• BSW
• Minimum of 1- 3 years experience in diverse clinical settings
• Licensed in the State of NJ
• Active American Heart Association Basic Life Support (BLS) certification
• Prior case management or discharge planning experience preferred
• Prior experience with Cerner EMR and All Scripts is preferred.
• Working knowledge of federal, state, and local laws that govern health care and case management.
• Knowledge of community resources.
