Social Worker (BSW) Job at Appalachian Regional Healthcare System
Position Summary
Under the supervision of the Case Management Manager, the Social Worker (SW) serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with a special focus on discharge planning, psycho-social needs assessment, care/resource management, and provider/insurance issues. Collaborates with the physicians, patients, their family members, and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family, and provider satisfaction, optimize systems/resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services.
The Social Worker’s decisions and actions with patients and families are determined in an ethical manner practicing compassion and respect for the inherent dignity, worth and unique attributes of each person. Effectively communicates with patients/families, visitors, colleagues, physicians, leadership, and volunteers. All areas of practice and care delivery are provided to achieve and maintain top decile results and outcomes for patient experience, quality, and patient safety. The Social Worker follows and adheres to ARHS policies and procedures, regulatory requirements and standards of behavior. The Social Worker practices within the practice and scope of their license.

Responds quickly and accurately to emergencies in the work environment and/or to changes in patient condition. Performs duties under general supervision. Always assists coworkers, patients, families, and visitors regardless of the patient assignment, so long as the assistance is in-line with their scope of practice and license.
Compensation is competitive and commensurate with education and experience. Hiring compensation ranges from $19.00 / hour to $27.55 / hour. Base rate is before shift differentials. This is a non-exempt position.
Minimum Education

Graduate of an accredited bachelor’s degree program in Social Work
Experience
-
Previous experience in case management preferred
Required Registration/License/Certification
-
Licensed in the state of North Carolina preferred
-
Basic Life Support (BLS) from the American Heart Association is preferred.
Knowledge, Skills and Abilities

Knowledge of and works within the scope of practice of a social worker

Ability to communicate courteously and effectively with the public, patients, family, staff, and physicians

Acts within the chain of command, utilizing positive verbal and non-verbal skills.

Competent in providing safe and effective patient therapy within limits of certification and hospital policies and procedures

Knowledge of the continuum of care provided

Ability to demonstrate exceptional customer service to patients, family, staff, and physicians
