Responsibilities
Our Director of Clinical Services at Rockford Center is seeking a
Clinical Case Manager with a passion for working with our patients. The Clinical Case Manager will be responsible for providing efficient and effective case management and coordination of services for youth, adolescents, and adults in our behavorial hospital setting.
Key Responsibilities:
- Meet/ Greet new patients
- upon admission to facility within 72 hours of admission to the facility
- Provide individual, group, and family therapy
- Collaborate with other staff in coordinating therapeutic services
- Coordinate activities to ensure that treatment plan goals and objectives are met
- Maintain accurate, complete, timely, and appropriate clinical records and other required information

Qualifications
REQUIREMENTS:
- The Clinical Case Manager must have a minimum of a Bachelor degree in Psychology, Counseling, Social Work graduated from an accredited college or university, preferred is a Master’s in Psychology, Counseling, Social Work, or related Human Services Field. One year experience working with children is required.
- Valid DE State Driver License with appropriate Endorsements.
- Ability to read and interpret reports and other legal documentation. Ability to write reports and ability to effectively present information and respond to questions from administration, DCS, children and their families, and general public.
- Physical Demands: Must be able to stand for extended periods of time up to and including 8 hours, lift 75 lb. assisted or unassisted, must be able to stoop, bend, reach objects overhead, attend and manage meetings.
