As a Social Worker Assistant, You Will:

• 
  The Social Worker Assistant will assist the Health Home Program manager in all facets of the program but particularly with the clinical aspect; including training, crises consultation, monitoring clients' services, chart compliance, intakes, assessments, etc. and participates in staff meetings, inter-agency meetings.
• Provide ongoing services based on program structure involving assessments, treatment plan, reassessments, etc. for client, family and collaterals.
• Maintains linkages with various social, health, education, legal, community, housing, drug rehabilitation and other services and represents program when necessary.
• Conduct Home Visits, accompany patients to their appointments, coordinate patient's appointments
• Carry a caseload of 50+ cases; client population: Low Income families, substance abuse, HIV population.

As a Social Worker Assistant, You Need:

• 
  Bachelor's Required
• Master's Preferred
• Experience working with Low Income families, substance abuse, HIV population.
