Social Worker, Aging – County of Bergen – Department of Human Services

Under direction, performs office and field work necessary for providing information, referral, and social services for older people; does related work as required.

Knowledge and Abilities:

  • Based on request from individual or on referrals of families, friends, institutions, or other community sources, visits older people in their homes, boarding homes, or wherever located.
  • Interviews clients, family members, and/or referral source to obtain information about the immediate problem and the client's resources and ability to deal with the problem.
  • Explains and describes facilities and services that are available to older people designed for the unique needs of the elderly.
  • Develop relationships with relevant agencies including Adult Protective Services (APS), Alternatives to Domestic Violence (ARP), Prosecutor’s Office, Mental Health Professionals, Advance Practice Clinician (APC) providers
  • Gather and disseminate materials and training opportunities to Senior Services staff and providers
  • Act as “victim advocate” for seniors who are victims of abuse or at-risk of abuse
  • Provide support to seniors in accessing services of APS, ADV, Family Guidance and other organizations that respond to abuse and crisis
  • Provide Outreach and Community Education on recognizing the signs of abuse and neglect, understanding how the system works, how victims retain their rights, and what resources are available
  • Ability to communicate effectively with elderly people, to obtain their respect and confidence, and establish and maintain constructive relationships with them
  • Knowledge of the physical, social, psychological, and economic problems of the elderly.

Schedule:

  • 40 hours per week, Monday – Friday

Education and Licensure:

  • Licensed or certified social worker in accordance with N.J.S.A. 45:15BB-1 et seq. and N.J.A.C.13:44G OR
  • Registered nurse in accordance with N.J.S.A. 45:11-23 et seq and N.J.A.C. 13:37 OR
  • Graduate of an accredited college or university with a bachelor’s degree or higher, in a health-related field or behavioral science field and one year of compensated work or internship experience with older adults or persons with disabilities in an institutional or community setting
  • The license, certification and educational requirements for care managers may be waived for any care manager supervisor or care manager employed in such position prior to December 31, 2017

Other Requirements:

  • Bilingual preferred English/Spanish
