Luther Crest, A Diakon Lutheran Senior Living Community — Located in Allentown with easy access to Route 78, Route 22 and Route 476. Luther Crest is a large Continuing Care Retirement Community with a skilled nursing and rehabilitation center, along with a personal care community. Luther Crest is seeking skilled and caring individuals to be part of Diakon’s Many Hands One Heart team. Employees are able to use Luther Crest’s state-of-the-art Wellness Center including a fitness center, spa, and indoor heated Olympic-size pool.

Hours: 8 days of work in a 2 week pay period (64 hours)

• Full-Time, Day Shift
• Part-Time, Day Shift

General Responsibilities:

• Responsible for providing social services to residents.
• Researches and documents new resident/client's social history and completes an admissions note; schedules conferences with residents' or clients' families as required.
• Provides direct services to residents and families including social assessment, casework services, and discharge planning.

Education/Training: A Bachelor’s degree in Social Work or Human Services; including but not limited to sociology, gerontology, special education, rehabilitation counseling and psychology.

Experience: One (1) year of supervised social work experience in a health care setting working directly with individuals required.
