Position Title: Social Worker
Supervisor’s Title: Administrator
General Statement of Responsibilities: The Social worker is responsible for the provision of supportive, social services to residents and their families.
Occupational Requirement: Possession of Bachelor’s Degree in Social Work or Bachelors in related field
Major Job Duties:
1. Assess resident’s psychosocial needs, and assist in the development of a treatment plan to meet the resident’s needs.
2. Visit residents and develop a supportive relationship with residents as well as their families.
3. Provide supportive counseling to residents and their families as needed.
4. Assist in linking residents to community and medical services which will benefit the resident. Provide information to service providers to ensure coordination of services, and establish effective working relationships with community medical providers and service agencies.
5. Participate in the interdisciplinary planning process, and attend meetings as necessary.
6. Documentation of services to include progress, declines, and changes of residents’. Ensure resident’s care plans are complete and in compliance with Federal, State, and Local regulations.
7. Develop and produce reports regarding resident’s services and needs as requested and required. Assist in the completion of the Minimum Data Set and resident assessment protocol reviews as needed for each resident.
8. Provide administrative case management services to residents which will include assisting residents and their families to complete necessary paperwork, obtaining required signatures.
9. Assist in residents’ discharge planning process; and coordinate the process.
10. Attend in-service, staff development/training events, and applicable meetings as appropriate.
11. Perform other duties as assigned by the Social Servicer Director Administrator to ensure the delivery of a comprehensive Social Service program to residents and their families.
Knowledge, Skills, & Abilities In: Social Work practice, effective counseling techniques; behaviors and issues of geriatric patients and effective interventions; community skills; working with families; ability to maintain confidentiality; effective problem solving skills; treatment planning and working effectively with a treatment team.
Physical Requirements/Abilities: Must be able to regularly in performance of duties, stand, sit, walk, kneel, and bend. Have visual and hearing acuity sufficient to assess residents’ needs.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
