Service Coordinator – Case Manager

We are growing and looking for employees who are enthusiastic about working in a position that enhances the lives of seniors. This position requires Spanish fluency.

Interview new participants and establish treatment goals in accordance with the Individual Plan of Care. Complete assessment documentation and assist with arranging and monitoring community-based services.

· Assess, plan, implement, coordinate, monitor, and evaluate the services required to meet a participant's needs

· Participate in the completion of comprehensive assessments and plans of care

· Maintain and create service coordination notes in the participant's health record on an ongoing basis

· Inform and refer participants to available community services and provide coordination of the medically related physical, mental, and behavioral, service needs of a participant

· Assist with any personal, social, family, or adjustment problems the participant may experience at the Program

· Refer the participant or family to the appropriate community resources if the participant or the participant's family requires specialized counseling

· Coordinate services if a participant requires assistance fromother community agencies, and no agency is acting as coordinator of services for that participant

A minimum one year of experience working with adults in a professional capacity is required. A degree or certificate in human services is preferred.

The pay range for this position is $19 to $21 per hour

We are a Latino Adult Day Health Program that offers older adults and people with disabilities a safe, secure, and caring environment where they receive nursing, social, physical, nutritional, and recreational services designed to promote health and well-being. Participants relax in warm surroundings with old and new friends and participate in a number of fun and therapeutic activities before returning home.

Our unique approach provides culturally appropriate activities, meals and environment and specially selected staff that understands this population’s needs and culture in the Roxbury neighborhood.
