Salud Community Health Worker- Bilingual Job at One Community Health

The Salud Community Health worker provides basic health and self-management education to patients with chronic and lifestyle-related conditions such as type I & II diabetes, gestational diabetes, prediabetes, elevated Body Mass Index (BMI), hyperlipidemia, congestive heart failure (CHF) and hypertension under the direction of the Preventative Health Nurse Primary Care Provider (PCP), and/or Preventative Health Manager. Patient support includes individual and group education, case management, and referrals, and is provided at One Community Health (OCH) health centers, patient homes, and outreach settings as needed. Education provided utilizes person-centered, popular education, and motivational interviewing techniques. This position also participates in Quality Improvement initiatives related to chronic disease management and is integral to PCPCH teams, serving as a link between the community/patient perspective and clinical services.

Minimum Qualifications:

  • Strong written and oral communication skills including proficient reading, speaking, and writing in English and Spanish.
  • High School graduate or GED
  • Able to give presentations and provide instruction to patients and groups.
  • Able and willing to travel to: other OCH sites, community activities, trainings, home visits, and/or other locations as designated.
  • Possession of a valid driver’s license, auto insurance, and reliable transportation.
  • Capable of collecting physical assessments: blood pressure, fingerstick blood glucose, and height/weight.
  • Knowledge of medical terminology and common medications for chronic disease.
  • Understands Community Health Worker scope of practice and comfortable asking for assistance regarding medical recommendations Knowledge of medical terminology
  • Knowledge of community resources in OCH services areas.
  • Able to prioritize and organize work according to multiple and competing timelines, and meet timelines for team and organizational goals safely and with high level of quality.
  • Successful relationship management skills involving multidisciplinary teams in a diverse, multicultural environment.
  • Able to use initiative and good judgment to resolve problems and challenges.
  • Able to inspire trust and confidence and communicate effectively with internal and external audiences.
  • High level computer skills with Microsoft Office applications, Word, Excel, Outlook, Epic, and other systems.

Preferred Qualifications:

  • Some advanced education such as graduate of an accredited Medical Assisting program.
  • One (1) year of customer service-related experience working with the general public; or
  • A combination of equivalent education and work experience.
  • Certified Community Health Worker
  • Experience working with low-income and diverse populations
