The RN renders professional care in accordance with the physician/provider treatment plan and the nursing process. S/he ensures that compassionate, engaged care meets or exceeds quality standards creating a positive and supportive experience for the patient, family and other caregivers. Care is provided in an efficient and safe manner consistent with the Maryland Board of Nursing licensure and Hospice of the Chesapeake (HOC) standard of care. The RN recognizes the legal responsibilities as specified by law and abides within those responsibilities. The Registered Nurse (RN) is a professional caregiver who assumes responsibility for any nursing visit provided in all patient care settings, including but not limited to skilled and crisis nursing visits, death visits, admission, liaisons, continuous care and GIP.

This position can also assist/provide Triage Support. The Nurse would manage in-coming calls and provide answers to questions, locate the appropriate person or resource and provide comfort and manage symptoms until a nurse can be dispatched and arrive at the patients home. Facilitate communication between patient care teams and on call.

  Follows all prescribed and accepted standards in providing patient care.
• Completes point of care documentation
• Instructs patients and families in managing symptoms, triages all calls and routes to appropriate resources.
• Attends and actively participates in at least 80% of Team meetings.
• Assumes primary responsibility for HOC total census during assigned shift, including the triaging, assessing, planning, implementing and evaluation phases of the nursing process. Maintains regular communication with triage, clinical managers, physicians, and members of the interdisciplinary team concerning patient/family care needs and response to interventions.
• Maintains open and professional communications in all interactions. Intercedes as necessary to promote positive, constructive interactions between staff members/departments/others.
• Demonstrates responsibility for scope of position/own standard of practice. Maintains required skills, licensure, regulatory requirements, and credentials needed to perform assigned duties.
• Demonstrates full knowledge of current position's, and department's, relationship to flow of patient services and/or care plan of the patient. Demonstrates understanding of all other services of the organization.
• Assist with triage when needed.
• Participates in and supports continuous process improvements.

QUALIFICATIONS

Possesses a current Maryland Board of Nursing (RN) license and thorough knowledge of nursing principles.

Demonstrated ability to provide quality patient care in a compassionate manner. Possesses ability to assess patient’s needs, e.g. physical, teaching, mental and spiritual and help create the plan of care that supports meeting all identified needs.

Must have excellent communication skills both oral and written. Good command of the English Language.

Must be able to perform the essential functions of the position with or without reasonable

Strong and creative, problem-solving skills, with the ability to prioritize, a must. Seeks assistance from Team when appropriate.

REQUIREMENTS

Must maintain a valid driver’s license with a clean driving record.

Must be able to safely move and lift 50 pounds.

Compliant with health screening and vaccination requirements of organization and position, including TB screening and Influenza vaccination annually.
