Job Summary:

The RNs provides skilled nursing, care, and case management for assigned patients in their place of residence; coordinates and integrates services to ensure continuity of care; facilitates the achievement of quality, clinical and cost outcomes; negotiates, procures and coordinates services and resources needed by the patient or family; creates opportunities and systems to enhance outcomes; and uses the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter-disciplinary team approach.

  • Provides skilled nursing to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician. Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals.
  • Observes, assesses and evaluates patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications. Assures continuity of care through effective communication, information exchange and case conferences with other disciplines, community health, support and social agencies.
  • Works in tandem with the Clinical Team including Home Health Aides, Volunteers, Chaplains, Social Workers, Therapists and other nursing staff.
  • Maintains accurate, current and complete patient records in accordance with Agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations.
  • Participates in team scheduling coordination and “on-call” rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required.
  • Participates in quality improvement activities and attends in-service training as assigned.

Minimum Qualifications:

Qualifications include a current RN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation. High speed Internet needed for remote work. Positions assigned to the Missoula office require union membership.


Schedule:

Specific work days and work hours are as coordinated with your manager. This is a local field position; successful out-of-state candidates will be expected to relocate within driving distance of the main office in Missoula, MT.
