Together Homecare's newest office in Valparaiso IN is seeking a RN Registered Nurse Case Manager to join our office and it's mission in being a top community resource for in home healthcare for the residents of Valparaiso and surrounding areas of Northwest, Indiana

The RN Registered Nurse Case Manager is responsible for the clinical oversight of our patients from admission through discharge. In this role, you will be managing a case load of patients, evaluating patient needs, supervising employees, and ensuring quality care is delivered. This role is performed both in the office through telemedicine and in the patients home through supervisory, recertification, and start of care visits. The role also has the capability of being performed hybrid in the office and from home.

Responsibilities:
- Manage all elements of the course of homecare, including care planning, goal evaluation, coordination of care, and care supervision/evaluation
- Ensure timely and accurate communication and documentation, both in the office and in the field
- Provide education to team members, patients, families, caregivers, field staff, and referral sources
- Participate in frequent care coordination and transition management activities across the healthcare continuum
- Ensure effective communication to patients, family members and physicians
- Participate in staff meetings, required in-services, and agency performance improvement activities
Requirements:
- Current license as a Registered Nurse in the state(s) of practice.
- One (1) year supervisory experience in an acute care setting or equivalent experience.
- Complies with accepted professional standards of practice.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Knowledge of home care federal and state regulations.
- Knowledge of reimbursement sources and documentation requirements within home health care.
