The MDS Coordinator is a Registered Nurse who is responsible for the timely and accurate completion of the MDS treatment assessment tool. This critical team member must be able to deliver high-quality care combined with the skills to manage data and care plan details. The MDS Coordinator will review care plans and delivery for factors specific to geriatric residents (i.e., physical, cognitive, and socialization factors). This tracking ensures that Wake Robin is in compliance with our high standards of care for our residents.
Key Qualities:
- Excellent communication.
- Embrace a collaborative and team approach by working with:
- Social Workers to schedule and provide resident/family care planning meetings.
- Nurse managers to develop and implement care plans.
- Nursing team to regularly monitor quality indicators and measures for
benchmarking against state and national data.
- Comfortable using data to track and trend care practices.
- High attention to detail to audit medical records for accuracy, appropriateness, completion and follow up.
Education and Experience:
- Current VT license as a Registered Nurse
- Minimum of 2 years in a supervisory position in a long-term clinical care setting
- Minimum of 1 year managing MDS and SNF Medicare requirements. Please note that while we prefer a candidate with a background/familiarity with MDS regulations, coding and care planning, and compliance, we will train the right candidate on the job.
Compensation:
Starts at $77,459.20/year, increases depending on experience.
Must be vaccinated for COVID.
