Major Responsibilities:
Provides work direction to the clinical and support staff with responsibility for the coordination of resources to efficiently support procedures, day-to-day operations and the smooth flow of patients through the department.
Provides support for direct patient care by performing the nursing process to deliver safe, therapeutic, and quality care through assessment of patients, establishment and implementation of the nursing care plan, and evaluation of clinical outcomes when necessary. Promotes an environment conducive to the delivery of safe, efficient and quality patient care, which may include performing patient rounding on a daily basis.
Coordinates the needs of the patient care team by initiating service complaint follow-up when necessary and promoting communication among patients, families, hospital departments and medical staff to optimize the interdependence of team members.
Acts as a resource to employees, which includes training/orienting, providing day-to-day work direction, and giving input on performance. Assigns, monitors, and reviews progress and accuracy of work, directs efforts and provides guidance on more complex issues in collaboration with the unit leadership.
Assists with human resource responsibilities, which may include: interviewing and selection of new employees, resolution of employee concerns, and employee moral or other duties as assigned in collaboration with unit leadership.
Monitors employee assignments and works on assignments for the unit. Evaluates staffing levels and makes adjustments as necessary to maintain productivity level established for assigned area(s).
Monitors the quality of care provided. Assists in ensuring policies and procedures are followed in compliance with the medical center, the Nursing department, and regulatory and peer review agencies.
Provides input to budget and ensures adequate supplies and equipment and inventory are available for both the delivery of safe patient care and a safe work environment.
Works with management in developing new and existing programs and services.
Works with vendors for requested products and coordinates in-services for equipment. Participates in product trials, and submits recommendations to management in collaboration with the unit Educator or CNS.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure:
Nurse, Registered (RN)
Education/Experience Required:
Bachelor's Degree in Nursing or Health Related Field; or Associate Degree and enrolled in BSN program within 2 years of hire and complete BSN degree within 5 years Typically requires 3 years of experience in clinical nursing within an acute care setting.
Knowledge, Skills & Abilities Required:
Demonstrates a holistic approach to nursing practice committed to meeting the needs of the patient and family. Clinical and technical knowledge in area(s) of specialty. Ability to apply evidence-based interventions and nursing skills/technology to correlate to the patient’s response to an individualized plan of care. Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.

Team members will maintain annual educational requirements for hospital based certifications (e.g. DNV, CARF or others)

1. 12 Center is an 18 – bed respiratory/geriatric and medical telemetry unit with the capacity to care for ten ventilator patients. Our typical patient population includes adults with pneumonia, COPD, asthma, CHF, ventilators, and tracheostomies. We have two negative pressure isolation rooms and the capability to dialyze up to four ventilator patients when necessary. We also provide medical-surgical nursing care for general medical-surgical and medical telemetry patients. 2. We have a multidisciplinary team approach to patient care. Due to the nature of our patient population, we work closely with therapies (PT, OT, Speech, and Respiratory), as well as the Nutritionist. We also hold nursing rounds every weekday. The unit is often used as a steppingstone from the Intensive Care Units and other critical care areas as a result of the vent population. B. Department Objectives/Goals 1. The care we provide is holistic. This means that we understand people have physical, emotional and spiritual needs and their relations to God, they, their families and society are vital to health and healing. Types and Ages of Customers Served 1. 12 Center is an 18-bed medical telemetry unit with a specific focus on geriatrics, pulmonary rehabilitation, and the care of patients with lung disease as well as patients with other complex medical problems. 12 Center serves an adult population. D. Telemetry Appropriateness Criteria 1. Patients requiring medical telemetry monitoring 2. IVP medications permitted: Digoxin, Lopressor, Cardizem 3. IV drips permitted: Lasix, Bumex 4. 12 Center does not care for patients with post pacemaker placement or post cardiac catheterization patients E. Ventilator Guidelines and Criteria 1. Ventilator dependent patients require a tracheostomy 2. Hemodynamically stable 3. Positive End Expiratory Pressure (PEEP) less than or equal to 5 (unless approved by medical designee of the unit) 4. Tolerate FiO2 less than or equal to 60% a. Exception: Patient may be on 100% FiO2 if there is a physician order for hospice/palliative/comfort measures b. In the event of a Rapid Response call, PEEP and/or FiO2 can be adjusted to a higher setting to accommodate the patient’s condition. If there is no improvement noted in the patient’s condition one hour on the higher settings, this would warrant a transfer to a higher level of care for further interventions and closer monitoring. c. Exception: If deemed appropriate by the critical care intensivist, a chronic ventilator patient may transfer to 12C on a higher FiO2 and/or PEEP setting. 5. Pulmonologist on consult to manage ventilator settings Hours of Operation: A. The 12 Center Respiratory Medicine Care Unit operates 24 hours a day, 7 days a week. Skill Mix: A. The 12 Center Respiratory Medicine Care Unit is staffed with a combination of Nurses and Nursing Assistants. Nurses range from novice to expert, inclusive of new graduate RNs and experienced RNs across all shifts. Nurses and Nursing Assistants are supported by ancillary department team members such as Respiratory, Physical, Occupational, and Speech Therapists. Core staffing and methods for determining and modifying staffing to meet patient care or process needs: A. Staffing Plan (see unit resource list) 1. Hospital staffing needs are based on the workload within each department and are maintained at a level that ensures a high quality of service while maximizing productivity. It is the responsibility of management to monitor the workload within departments and to make appropriate adjustments to staffing levels as workloads fluctuate. Each clinical unit has an established staffing plan that reflects the number of staff needed to provide optimal patient care. That staffing plan takes into consideration the patients served, the environment of care, the patient acuity, staff competence and the degree of supervision required. Staffing is monitored every 4 hours and adjusted according to need. Staffing plans, trends, and variances are evaluated annually and as needed by the Manager and the Director. B. Methods used to assess and meet patient care needs. 1. We have a multidisciplinary team approach to patient care. B. Extent to which available services meets patient care needs 1. Care provided is appropriate, courteous and of high quality. 2. Development and maintenance of a relevant performance improvement plan. 3. Communication and reporting of performance improvement activities. 4. Identification of deficiencies and/or opportunities for improvement with development of appropriate corrective action plans. 5. Resolution of problems that impact on patient care delivery. 6. Resolution/Response of patient/family complaints. 7. Continuing education and in-service offerings, including Customer Relations and Patient Safety. 8. Patient Perception. 9.
