Responsibilities:

• Demonstrates excellent verbal and written communication skills with patients/families and fellow peers
• Demonstrates leadership skills in development and updating the patient's plan of care.
• Demonstrates empathy in the delivery of patient centered care
• Completes, maintains, and submits accurate and relevant timely clinical notes regarding patient's condition and care given.
• Provides professional nursing care buy utilizing all elements of the nursing process.

• Assess and evaluate patient status.
• Completes nursing assessments utilizing critical analytical skills with strong support from the IDG team
• Individualized orientation based on previous experience
• Initiate and revise patient plan of care.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
• Participates in on-call duties as defined by the on-call policy.

Registered Nurse - RN Case Manager Qualifications:

• Licensed as a Registered Nurse in the state of Minnesota and/or Wisconsin
• CPR certified
• Prior Nursing experience in a TCU, Hospice, or Home Health setting preferred
• Valid Driver's License and reliable vehicle with current insurance
• Excellent at planning, organizing, and communicating
• Demonstrated ability to work autonomously
