The position:

We are looking for a full time RN Care Managers. The ideal candidate for this role has strong field care management experience working with members with acute and chronic conditions and is responsible for conducting monthly face to face visits, assisting in coordination of comprehensive care. When not in the field conducting visits, this role is expected to report to the office to participate in and conduct virtual care calls/visits, follow up correspondence, internal reporting, collaborative care meetings, and all routine team meetings.

Requirements:

• Current and valid State of Nevada RN license and graduation from an accredited Registered Nursing program required.
• Prior care management experience in the field required.
• Experience working with adult, older adult, geriatric population preferred.
• Experience and understanding of population health management preferred.
• Driving is a requirement for this position. Current Nevada driver's license and proof of insurance required.

Main Job Functions

• Maintain case load of between 40-250 patients or based on the needs of the client and/or contractual obligation.
  • 100% Office-Based, virtual care management role requires 150-250 patient case load
  • Hybrid of Office-Based & field care management role requires 40-60 patient case load
• Conduct face to face or virtual visits at a minimum of every 30 days for assigned patients enrolled in Intensive Care Management.
  • Provider may conduct increased face to face or telephonic visits based on the needs of the patient and or contractual obligation.
• Complete Clinical and Social Assessments (CSA) to develop care plan upon initial start of care visit with patient.
• Assist in coordination of comprehensive care; including transitions of care, obtaining referrals to other interdisciplinary services (Home Health, Palliative Care, Social Services) to help prevent further need for acute care utilization.
  
• Prepare for ICT rounds
• Collaborative care coordination with colleagues in other departments to ensure maximum efficiency of visits
• Quality Gap Closure as needed and referral to other providers and programs internal/external, as appropriate
