JOB SUMMARY:

The primary purpose of the position is to coordinate all service delivery at Genesis Group Homes. These functions include; coordinating with the Interdisciplinary Team, licensing, conducting internal audits, assuring service satisfaction, managing the organization's procedures for vulnerability and mandated reporting; and managing current and past individual records. Additionally responsible for overall program services, including: assessments, staff training, finances & maintaining compliance records. This position provides strategic direction to the programs and supervises the Residential Services Managers, Crisis Services Senior Manager, Nurse Manager, and Occupancy & Licensing Manager.

This position reports to the Chief Clinical Officer (CCO). The position is classified as exempt.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Oversees the daily workflow of the residential & crisis service delivery department.
  • Evaluates current services and assess the need to extend, modify, or end services for individuals served.
  • Implements and monitors service delivery budget.
  • Facilitates the implementation of professional development and training for residential service delivery staff.
  • Responsible for overall programming including individual assessments, staff training, finances & maintaining compliance records.
  • Responsible for program data, program evaluation and in coordination with the CCO , evaluates program outcomes and engages in quality improvement projects.
  • Ensures the completion of SMA, IAPP, PAPP, SPA, PAPP, monthly reports, medication reviews, serious incident reviews, safety assessments, and similar operational reporting.
  • Creates action plans for all individuals before discharge to secure community support services and ensure appropriate discharge plans are written and executed.
  • Provides programming leadership to direct reports, particularly during crises to affect an appropriate and therapeutic solution.
  • Ensures licensing requirements are adhered to.
  • Promotes diversity and cultural inclusiveness at Genesis Group Homes.
  • Participates in group and team meetings, including Leadership Team meetings, as necessary.

Audits:

  • Plans and implements internal auditing and reporting system.
  • Designs audit tools and mechanisms to measure the effectiveness of the department's missions.
  • Develops plans to remedy deficient program operations or quality, or to enhance these where it is already satisfactory.
  • Coordinates policy, procedure and record keeping systems related to internal audits.

Service Satisfaction:

  • Identifies internal and external service satisfaction problems and recommends solutions.
  • Investigates received complaints, makes recommendations, and resolves them and documents outcomes.

Vulnerable Adults:

  • Coordinates policy, procedure and recordkeeping systems related to vulnerable adult reporting and follow through.
  • Coordinates the organization's response to County or DHS investigators.
  • Conducts internal investigations of reported incidents, reports conclusions and makes recommendations.
  • Responsible for ensuring investigation and reporting expectations are met.

Individuals with Developmental Disabilities Record:

  • Coordinates and manages the current and past individual records.
  • Assures the security and privacy of such records.
  • Coordinates all policy, procedure and record keeping systems related to individual records and storage.

Standards of Performance:

  • All policies, procedures, guidelines and practices meet or exceed requirements under local, state, and federal laws and regulations to retain compliance and licensure.
  • Risk management measures are implemented to avoid and/or ameliorate risky practices, non-practices/incorrect practices or violations of any sort.
  • All information is secure and safe.
  • Communications are correctly and promptly provided to appropriate persons.
  • All interaction and service delivery promotes customer relations.
  • Responsible for overall success of DHS & county licensing reviews.

JOB REQUIREMENTS:

  • Ability to move intermittently throughout the workday.

QUALIFICATIONS:

  • Master’s Degree in Social Work or related field preferred; Bachelor’s Degree in Social Work or related field required.
  • Four year’s experience in senior management position
  • Four years combined experience in the provision of services to persons with developmental or physical disabilities and four years of supervisor experience.
  • Knowledge of CRS, ICS, Crisis Respite, and other services for those individuals with other specialized needs including physical and intellectual disabilities, aging and traumatic brain injury.

PHYSICAL AND MENTAL DEMANDS:

While performing the duties of this job, the employee moves intermittently throughout the workday. The employee is regularly required to see or hear, with or without prosthetic devices, sufficiently to perform the duties and communicate effectively with staff, individuals, family members, management, agency personnel, and the general public. The employee frequently is required to use hands to finger, handle, or feel. The employee is frequently required to stand and walk for periods of time; frequently bend/stoop, crouch, and kneel. The employee must occasionally lift and/or move up to 50 pounds. The employee will frequently demonstrate restraints and lift persons or implement required therapeutic interventions, if required. Specific vision abilities required by this job include close vision, and ability to adjust focus.

WORKING CONDITIONS:

  • Is subject to frequent interruptions, imposed deadlines and frequent problem-solving activities.
  • May be subject to hostile and emotionally upset individuals, staff, family members, visitors and personnel from other agencies.
  • May be exposed to a variety of conditions, infectious diseases, odors, dust, etc. throughout the working hours.
  • Regularly travels in the community, drives a vehicle and transports individuals and will be asked to use your own vehicle (with paid mileage).
  • Is subject to flexible hours, including split shifts or varied days and times including mornings, afternoons, and evenings, holidays, weekends and overnights depending on coverage needs.
  • Work locations may vary according to the needs of the company and staffing requirements. May be assigned to work sites throughout the various counties contracted for services with the company, including reassignment to a different shift when the scheduled shift has been cancelled.
  • May be subject to physically aggressive individuals and may be required to participate in a behavioral restraint.
  • Carries and responds to cell phone during non-working hours.
