Job Title: Registered Nurse RN Hospice Case Manager

Job Summary: Our hospice organization is looking for a compassionate, skilled and highly motivated individual to join our team as a Registered Nurse RN Hospice Case Manager. The RN Hospice Case Manager will be responsible for providing comprehensive and compassionate end-of-life care to patients, assessing the physical, emotional and spiritual needs of patients and their families and developing personalized care plans that promote symptom management and comfort. In this role, the RN Hospice Case Manager will work collaboratively with an interdisciplinary team to ensure high quality and evidence-based hospice care.

Responsibilities and Duties:

• Conduct initial and ongoing assessments of patients, including physical, emotional and spiritual needs.
• Develop and implement personalized care plans that promote symptom management and comfort.
• Coordinate care with interdisciplinary team members, including physicians, social workers, chaplains, and volunteers.
• Provide education and counseling to patients and families on hospice care, symptom management, and end-of-life decision-making.
• Monitor patient progress and make appropriate changes to care plans as necessary.
• Facilitate communication between patients, families, and healthcare providers.
• Provide support and guidance to families during the grieving process.
• Maintain accurate patient records and comply with all regulatory requirements.

Qualifications and Skills:

• A valid RN license in the state of Virginia.
• Minimum of 2 years of clinical nursing experience, with preference given to hospice or home health experience.
• Graduation from an accredited nursing program.
• Strong communication and interpersonal skills.
• Ability to work effectively in an interdisciplinary team.
• Knowledge of hospice philosophy and end-of-life care.
• Strong problem-solving skills and ability to work independently.
