Registered Nurse - PACT I

Programs in Assertive Community Treatment (PACT) is an evidence-based outreach program to help people with serious mental illness integrate into the community. Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multi-disciplinary team for persons with a serious and persistent mental illness. A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists.

Requirements:

• Graduation from an accredited school of nursing
• One year experience working with consumers with mental illness
• Current license to practice as a registered nurse in New Jersey
• Valid NJ drivers license with less than 6 points

Responsibilities:

• Participate in multi-disciplinary treatment team for collaboration of client care
• Engage all PACT consumers in treatment planning and all areas of service delivery
• Provide direct care as delineated in the individualized CTP
• Assists consumers with obtaining basic needs and activities of daily living through modeling, direct assistance with ADLs and coaching
• Provides on-going assessment of client’s emotional well being, clinical symptomology, behavioral changes and medication compliance
• Provides psychosocial education for consumers and family members on mental illness, symptom management, medication compliance and life skills
• Provide crisis intervention as necessary
