This position will be responsible for performing the duties of supervision, evaluation of Caregiver/Personal Care Assistant (PCA) staff, personalized training, and oversight of our clients Home Care Services.

Essential Functions and Key Responsibilities

• Primary oversight and responsibility for clients with physical and/or psychological health needs.
• Provide training and ensure competency for the Caregiver/PCA according to the individual needs of the client and their care plan.
• Complete phone and in-home visits to clients according to state guidelines and provide oversight, evaluation, and supervision of both the effectiveness and satisfaction of services.
• Create, implement, and maintain an individualized plan of care related to client needs.
• Prepare interagency notes, monitor for change in client condition and assess for both physical and psychological needs of the client.
• Ensure that authorized client services are being provided according to agency standards.
• Monitor and ensure that services are being delivered in a caring, compassionate, and respectful manner that exceed industry standards.
• Appropriately document all trainings, communications, evaluations, etc.
• Maintain complete, accurate and timely clinical records/documentation on all clients according to agency policy.

Qualifications

• Must be a Licensed Social Worker or Licensed Registered Nurse in Minnesota. License must be in good standing, without restrictions, suspensions, or revocations.
• Complete and pass a state required background study.
• Have a valid driver's license and use of insured personal vehicle.
• Be able to lift, turn, position, and transfer up to 50 pounds.
• Demonstrate the ability to read, write and carry out both written and verbal directions in English.
• Comfortable with computers and technology.
• Experience in a health care setting, mental health setting or equivalent, home care or case management experience preferred but not necessary.
• Ability to organize workflow and prioritize daily schedule.
• Demonstrate exceptional professional judgment and can recognize health needs and safety concerns in the home.
• Feel comfortable working with clients of all ages and can adapt to a variety of working environments and community settings.
