Advantage is currently seeking a Full-Time Registered Nurse to join our vastly growing Home Health team in the Greensburg, PA and Surrounding Areas! This position is responsible for providing patient care in their home.

Receive the Support you Need and Deserve:

• We provide on the job training including Client Assessment, Care Planning, Intervention and Overall Supervision of Patient Care in the home.
• RN Mentor assigned to provide one-on-one support

Responsibilities:

• Conducting Start of Care comprehensive assessments and completing OASIS timely
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patient and provide proper reports to Case Manager
• Provides health care instructions to the patient/family as appropriate per assessment and plan

Qualifications:

An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
