Responsibilities:
- Assesses clients for need of in-home care and requests care.
- Coordinates with Nurse Coordinator and physician for all necessary visits to obtain/ maintain in-home care.
- Coordinates nursing care for the client once approved by the Department of Labor for nursing, home health aide, and applicable therapies.
- Maintains confidentiality of all office and client records per HIPAA guidelines and Privacy Act PHI.
- Maintains effective communication and collaboration with entire care team on behalf of the clients.
- Performs other duties as assigned.
What We're Looking For:
- Current registered nursing license in good standing.
- At least one-year experience in an acute/chronic medical field or related with good assessment skills.
- Management skills are a plus.
- Good organizational and time management skills to manage multiple clients at one time.
- Good working knowledge of computer and internet-based applications along with other office machines as necessary to complete job.
- Competent working knowledge of client-based equipment.
- Must pass a background check and sanction screening.
- Professional appearance necessary.
