Education/Credentials and Special Requirements: Registered NJ Nursing - RN license. Bilingual Spanish preferred. The successful candidate will be required to visit patient sites as need arises. Areas may include, but not limited to, the following counties: Cumberland, Salem, Gloucester, Camden, Atlantic and Cape May Counties.
Professional Experience and Skills:
- Ability to coordinate patient service
- Strong organizational and communication skills
- Must have a minimum of 1-year Home Care experience
Position Responsibilities:
- Visiting and Coordinating Services for patients in the community
- Coordination of Services
- Maintain regulatory compliance by completing timely visits and completing paperwork in a timely manner
- Compliance with Physician Orders/Plan of Care Required
