The Quality Specialist position provides outreach to patients to support HEDIS, HCC, and other quality improvement activities. The Quality Specialist position provides support to additional activities aimed at improving HEDIS, HCC, and other quality and care coordination activities. This includes but is not limited to identifying information in the medical record, providing information to providers and/or office staff and participating in team shadowing activities.

Responsibilities:
  • Outreaches patients to schedule appointments. This includes determining what barriers or obstacles exist that prohibit patient from making or keeping appointment and determine what resources may be needed or action that should be taken.
  • Reviews patient’s charts as needed to determine compliance based on specification or process outlined. Determine next steps based on chart review findings.
  • Reads, understands and applies HEDIS/HCC specification and/or policy as it relates to compliance and outreach.
  • Determines possible root cause to issue effecting patient compliance and presents issue and possible solution(s) to management
  • Conducts work essential to supporting outreach efforts, including but not limited to:
    • Collecting and verifying compliance data
    • Entering data
    • Summarizing findings
    • Recording the results of outreach
    • Reporting results to management
  • Educates providers and other staff on quality related activities (e.g. HEDIS, HCC)
  • Performs other job-related duties as required.
Qualifications:
  • High School diploma
  • Bachelor’s Degree preferred
  • A minimum of two years’ experience in health care setting required
  • Excellent interpersonal, communication, organization and writing skills with the ability to effectively prioritize work required
  • Ability to adjust quickly to change required
  • Commitment to enhancing patient experience required
  • Must have computer proficiency, with proficient use of Microsoft Word, Excel, and PowerPoint required
  • Knowledge of HEDIS quality measures required
  • A minimum of two years’ experience in quality related activities preferred
  • Knowledge of utilizing electronic medical records systems (such as EPIC) preferred
  • Experience with HEDIS preferred
  • Experience with HCC preferred
  • Experience with PCMH preferred
