Description:

The Visiting Nurse Association is currently seeking a detail oriented, self-motivated RN who enjoys making a difference in the lives of others for our Public Health Nurse -Shelter Homeless Review Team position. This position is part-time eligible 24 hours per week.

This employee takes a leadership role within the multi-agency Homeless Review Team (HRT). Provides nursing assessment, health education, referral, follow-up, case management and advocacy for clients identified by the HRT. Collaboratively develops an individualized plan for high-risk, homeless individuals with complex behavioral, mental, and medical health needs which will result in housing stability and end chronic homelessness in Omaha and/or Council Bluffs homeless shelters.

Typical duties include:

  • Assists with planning, implementation, and evaluation of shelter nurse program.
  • Assesses shelter environment to ensure safe practices and ability to provide first aid, increase treatment compliance and resolution of health problems. Serves as health liaison among client, staff, community agencies and medical providers.
  • Maintains clinical documentation of health assessment, progress, referral, and teaching of shelter clients. Collects and reports required statistical data.
  • Provides health assessment, referral, and follow-up to promote preventive health care and treatment for suspected health problems.
  • Provides health counseling, guidance, and education to promote preventive health care and enhance treatment compliance and resolution of health issues.
  • Refers clients to appropriate community resources and establish system for follow up.
Requirements:


  • Active Nebraska or other RN licensure in a compact state.
  • Current Certified Basic Life Support.
  • Graduation from an accredited School of Nursing. BSN preferred.
  • 1 year experience and knowledge in the principles and practices of community health nursing preferred. Knowledge of VNA services and community resources preferred.
  • Valid Driver’s License and Auto Insurance coverage.
  • Must have your own reliable transportation.
