POSITION SUMMARY*
JFCS East Bay is hiring a licensed clinical social worker with experience working primarily with older adults and their families to join our dedicated and experienced team of mental health and social service providers. This role will serve the Jewish and wider community living throughout Alameda and Contra Costa Counties, including working with Holocaust survivors. The psychotherapist/care manager offers a holistic suite of services, including short- and long-term 1:1 psychotherapy, support groups, care consultation and care management services. Our team employs a biopsychosocial, culturally and linguistically responsive, trauma-informed approach to mental health and other social services through a variety of modalities to meet the needs of our clients (e.g., psychodynamic, CBT, ACT, etc.). One-time care consultation and on-going care management includes services to support families in decision-making choices, connecting to available services, and coordination of care for older adults. This professional works as part of the collaborative Adult Services team of professionals and reports to the Director of Adult Services.

POSITION RESPONSIBILITIES*

Psychotherapy*
- Provide individual psychotherapy to selected clients identified as benefiting from mental health services.
- Assess and refer community members seeking counseling services, as needed.
- Facilitate psychoeducational/support groups.
- Participate in clinical consultations with colleagues; potential to supervise early career clinicians.

Case Management and Home Care*
- Provide consultation services to older adults and their families.
- Provide ongoing care management services to older adults and those with disabling diagnoses, including MS.
- Conduct assessments and develop care plans for clients, including measurable outcomes that are cost effective and reflect client preferences and values; in selected cases, implement and monitor care plans.
- Responding to the needs and challenges of home care clients, conducting home visits as required.
- Provide information and referral services to older adults and family members.
- Train, oversee, and support volunteers working with older adult clients.
- Provide trauma-informed care/social services to Holocaust survivors.
- Provide social services and financial assistance to low-income Jews in the community through our Jewish Community Safety Net.
- Management of the Jewish Community Safety Net Program including tracking, financial assistance, forecasting and planning for the future sustainability of this program.
- Assist in outreach and promotion of JFCS East Bay services for older adults.

Clinical Services Community*
- Document and track client needs and outcomes in the aggregate for agency-wide learning and reporting.
- Contribute to a supportive collegial environment for all clinical staff, focused on professional learning and collaboration.

REQUIRED MINIMUM QUALIFICATIONS*
- Licensed Clinical Social Worker
- Three to five years working with adults and older adults or equivalent experience. Geriatric social work knowledge, skills, and experience.
- Ability to communicate effectively with older adult clients, families, and other community representatives.
- Demonstrated capacity to be open-minded and solution-focused.
- Cultural competence in working with people of various nationalities, ethnicities, and religious affiliations.
- Superior communication and interpersonal skills while ensuring confidentiality.
- Excellent record-keeping and documentation skills, experience with electronic health records and case management systems, Medicare and MediCal systems.
- Competent computer skills including Microsoft Office
- Knowledge of challenges faced by Alzheimer’s disease and other dementias.
- Knowledge and practice of trauma- informed care.
- Collegial, team player who is ready to contribute to a positive clinical services environment for staff and clients alike.

- Knowledge of East Bay aging-related community resources.
- Knowledge of Jewish culture and community.
- Knowledge of issues faced by Holocaust survivors.
- Experience with third party home care services.
- Knowledge of the dynamics of aging including physical changes, emotional dynamics, and familial interactions
- Bilingual.

COMPENSATION AND BENEFITS*
The compensation for this position will range from $75,000-$85,000 based on experience.
