Indian Creek Foundation, Souderton PA is seeking a* Program Supervisor (direct care)* who will direct all operational aspects of managing their assigned programs or facilities. She/he will oversee efforts aimed at active treatment and habilitation in all areas of development for each resident. She/he will work to create and maintain a safe, comfortable, home-like environment in the residential facilities and will facilitate a positive working relationship with families of people served in the community.
Education/Training:
- High School Diploma; GED. AA or bachelor's degree preferred.
- Valid Drivers License with a 3 year clear driving record
Skill(s):
- Ability to read and write in English, moderate mathematics skills required
- Strong interpersonal relations and communicative skills; auditory and visual skills.
Physical Requirements:
Ability to perform the physical duties required of Direct Care Worker. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care; and operate an vehicle.
The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds, including transferring of client.
Experience:
- Minimum of 2 years experience working in the field of intellectual/developmental disabilities.
- Supervisory experience of agency staff preferred.
Essential Duties (Residential)
- Supervises the day-to-day operations of assigned program(s).
- Trains, supervises and evaluates the performance of Direct Care Staff.
- Works with the Scheduler to maintain approved staffing ratios
- Assigns duties to Direct Care Staff in order to meet the needs of the individuals served in the program.
- Directs completion of, or completes, all required agency and regulatory documentation.
- Works to ensure that individuals receive needed healthcare services by scheduling medical appointments in conjunction with the Program Nurse and Scheduler
- Works with the Scheduler to arrange for staff to facilitate each individual's attendance at relevant medical appointments or personally completes the appointment as needed.
- Is knowledgeable of and fully implements all relevant regulations.
- Ensures compliance with regulations and agency policies by direct monitoring and assistance with required work duties in the program.
- Advocates for and encourages the growth and development of each individual served.
- Conducts regular group and individual staff meetings.
- Maintains Individual Funds and Agency Petty Cash in accordance with policy.
- Covers for other Program Supervisors on an as-needed basis.
Essential Duties (Supported Living)
- Supervises the day-to-day operations of assigned program(s).
- Trains, supervises, and evaluates the performance of Family Support Aides.
- Provides guidance to staff in helping individuals make independent decisions.
- Facilitates connection to community supports for the individuals served.
- Works with the Scheduler to maintain approved staffing ratios
- Assigns duties to Direct Care Staff to meet the needs of the individuals served in the program.
- Directs completion of, or completes, all required agency and regulatory documentation.
- Works to ensure that individuals receive needed healthcare services by scheduling medical appointments in conjunction with the Program Nurse and Scheduler
- Works with the Scheduler to arrange for staff to facilitate everyone's attendance at relevant medical appointments or personally completes the appointment as needed.
- Is knowledgeable of and fully implements all relevant regulations.
- Maintains accurate and timely documentation of individual's progress towards outcomes.
- Ensures compliance with regulations and agency policies by direct monitoring and assistance with required work duties in the program.
- Advocates for and encourages the growth and development of each individual served.
- Conducts regular group and individual staff meetings.
- Maintains Individual Funds and Agency Petty Cash in accordance with policy.
Essential Duties (Home Based)
- Provides supports and services through home visits, observations, and regular monthly meetings.
- Trains, supervises, and evaluates the performance of Community Based Support Staff.
- Reviews Progress Notes to ensure accurate completion.
- Communicates regularly with families to evaluate effectiveness and quality of services.
- Documents service provision though monthly and quarterly reports.
- Schedules services with families and support staff.
- Is knowledgeable about the agency's service offerings and promotes these offerings in order to generate new or increased business.
Other Duties
- Attends annual ISP meetings and other relevant meetings as assigned.
- Participates in the development of individual goals and outcomes for each individual.
- Participates in the On-call Supervisor rotation.

Pay: $34,012.65 - $50,000.00 per year
