Program Coordinator - Blended Case Management Job at Glade Run Lutheran Services
CORE COMPETENCIES:
- Oversee the insurance authorization process
- Develop and maintain positive relationships with referral sources and payers and actively participates in marketing the program to increase community awareness of the agency and as a provider.
- Collaborates with the Central Referral Department to expedite referrals to intake.
- Maintains a small caseload of consumers to meet agency productivity expectations of a minimum of 40 units per week.
- Fills behind blended case management to ensure client needs are met in the event of employee vacancies, illness, or approved time off.
- Provides monitoring and supervision to staff to ensure compliance with documentation standards to include intakes, assessments, services planning, service delivery documentation and discharge planning.
- Provide direct care and on-call services as needed
- Assists the Program Manager with oversight and supervision of BCMs during individual and group supervision.
- Assists the Program Manager with on-boarding, training of new employees
