REPORTING RELATIONSHIP:

Supervised by: Durable Medical Equipment Team Lead and Hospice Manager

Positions Supervised: N/A

Interrelationships: Patients, family, IDG and other health care team members

JOB SUMMARY:

Serves as a member of the Hospice IDG. Provides durable medical services to the terminally ill Hospice patient as needed, under direction of the Hospice RN. Provides equipment delivery/pick-up, set-up/take down, and instruction with understanding of the patient and family on equipment use and safety.

QUALIFICATIONS:

Educational/Degree: High school graduate or GED, preferred.

Training/Licensure: Completes Hospice Orientation program.

Knowledge/Skills/Ability: Willingness to work as a Hospice team member. Demonstrates knowledge of how equipment is installed and works and has well-developed communication skills.

Experience: Previous experience with durable medical equipment set-up/take-down and delivery/pickup, preferred.

Physical Requirements:

Requires considerable physical effort that includes kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient and/or equipment. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.

Mental Requirements:

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Be able to tolerate exposure to elements, adverse environmental conditions and hazardous materials.

Transportation:

Must have a current valid driver’s license, auto liability insurance and reliable transportation.

Essential Functions:

1. Serves as a member of the IDG as requested.

2. Provides equipment delivery/set-up and pick-up/take down at the patient residence under the direction of the Hospice RN per MD orders.

3. Disinfect and maintain equipment per manufactures’ written guidelines before delivery and with pick up.

4. Fix or report to DME Team Lead equipment in need of maintenance or broken equipment.

5. Communicates at the appropriate level with each patient and family.

4. Documents services provided to each patient and family.

5. Coordinate efforts with DME team if not able to provide assigned services.

6. Consult with team members for issues relating to the patient/family plan of care.
