Population Health Coordinator Job at Ryan Health
Position Overview:
The Population Health Coordinator is responsible for supporting practices and the population health team by facilitating the development, rollout, and maintenance of clinical programs to ensure the provision of high quality, service-orientated patient care and supportive services. Under the direction of the Associate Director of Population Health, this position will support the Care Management & Quality Programs, as well as help strengthen Ryan’s care delivery model in pursuit of clinical excellence. This work is carried out in support of the mission and goals of the Ryan Network.
Essential Functions:
- Assists Population Health Department leadership with the implementation and maintenance of clinical programs that support department’s initiatives
- Supports site implementation leads to ensure adaptation of new workflows and initiatives
- Evaluates process and outcomes data with site teams and Ryan leadership which serves as documentation of re-design efforts and tracks implementation progress
- Manages and monitors cross functional collaborations to develop, implement and maintain programs aimed at addressing social drivers of health (SDoH) across the network.
- Collaborates with QI analyst and Informatics Analyst to develop, generate and present data to maximize performance for Ryan Health’s population health efforts.
- Collaborates with QI analyst and Informatics Analyst to develop reporting and analyses to support strategic and contractual requirements for internal and external stakeholders.
- Develops in collaboration w/ Informatics and dashboards or other data visualization methods to display compliance.
- Works with various site and departments across Ryan Health to develop and document common methodologies, definitions, and workflows for reporting, ensuring accuracy, eliminating duplication of effort and providing a forum for sharing ideas and creative problem solving.
- Support Ryan’s MCOs’ Quality Incentive Programs to enhance clinical outcomes and maximize revenue potential.
- Assists the Population Health Department to internally publish best practices, clinical workflows and educational material to improve clinical measure outcomes for the network and QI Repository.
- Utilizes reports and populations health platforms to determine trends and status’ of projects
- Supervise the Department’s Interns and/or AmeriCorps
- Support dissemination of patient campaigns via our patient engagement platform
- Other duties and/or projects as assigned.
Other Responsibilities
- Participate in the site-specific and Network-wide Quality Assurance/Performance Improvement Committee.
- Participate and present at Board QI Meetings and other external meetings as needed.
- Knowledge of clinical measure regulatory standards and remain up-to-date on healthcare policies.
- Report any unusual occurrences or issues to Administration in a timely manner.
- Attend all meetings and/or training sessions as required
- Immediately report any unusual occurrences to supervisor
Job Qualifications
Minimum Experience and Skills Required:
- Two years of experience in a health care or not-for-profit position.
- Two years of experience with program management
- Possess skills in data analysis predominately in healthcare and/or clinical environment
- Strong analytics and problem-solving skills
- Knowledge of Improvement methodologies
- Self-learner, strong motivation to get to the bottom of problems
- Experience in effective data visualization and data analysis.
- Excellent research, writing and communication skills.
- Strong presentation skills.
- Ability to work collaboratively with other staff, specifically program staff.
- Ability to manage several projects simultaneously, and meet strict deadlines.
- Ability to organize and package information in a concise and attractive manner.
- Proficient in excel
- Familiarity with visualization tools such as PowerBI
Education, Licenses and/or Certification Required:
Bachelor’s Degree.
Preferred Qualifications:
- Master’s Degree in Public Health, Public Administration, Urban Planning, or related field.
- Familiarity with Healthcare Effectiveness Data and Information Set (HEDIS) measures.
- Experience in community-based settings.
- Bilingual: English/Spanish.
