Physical Therapist Assistant position summary

Provides physical therapy services to the organization’s clients under the guidance and supervision of the Physical Therapist, and in accordance with a written plan of treatment established by the physician and physical therapist.

Essential Functions

  • Provides physical therapy services to clients according to a physician’s written order and therapist plan of care.
  • Implements the care plans for clients developed by the supervisory Physical Therapist.
  • Provides direct physical therapy treatment to clients as assigned.
  • Instructs clients and family members in the use and care of therapeutic appliances and devices, such as wheelchair, braces, etc.
  • Assists in assessing the client’s need for physical therapy devices and interventions.
  • Reports to physician, PT, RN or branch management as necessary regarding the client’s response/reaction to treatment or a change in condition.
  • Discusses with and makes suggestions to the physical therapist regarding modification of equipment, appliances and the physical environment of the client’s home.
  • Instructs the client and family members in appropriate procedures and supportive activities for client, and evaluates follow-through by client and family members.
  • Accurately documents observations, interventions and client response pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system toward goals, and contacts with physicians, other agencies and/or community providers.
  • Submits/transmits documentation in a timely manner.
  • Works with scheduling to assure visits and client care coverage is scheduled in accordance with the plan of care, and maintains an appropriate productivity level.
  • Establishes and maintains consistent communication with the physical therapist and other agency staff to discuss client physical therapy goals, plan of treatment, services provided, client/family responses and discharge plan.
  • Attends case conferences when appropriate, initiates physical therapy supervisory meetings/visits, and one-on-one conferences with physical therapist as needed.
  • Provides support and encouragement to clients and families
  • Establishes and maintains a professional relationship with clients/families.

Minimum qualifications

  • Graduation from a two-year college-level program approved by the American Physical Therapy Association.
  • Minimum of six months experience under a qualified Physical Therapist.
  • Home health experience preferred
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
  • Must accept responsibility for maintaining clinical practice skills learning and adhering to agency’s policies and procedures on an on-going basis.
  • Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
  • Must have current Driver’s license and reliable transportation to and from work site.
